If your child is a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your child's medical needs more quickly than completing them on your arrival. Payment at the time of treatment is advised. For your convenience, we accept Visa, Amex, and Master. We take cash and credit card. We can also arrange in-house financing for uninsured patients. Please feel free to ask our team.
We are in-network providers with many of the PPO insurance plan including: Aetna, Anthem, BCBS, Cigna, ConnectiCare, Guardian, MetLife, United Concordia, United Healthcare, and many more. We also accept HUSKY. If you do not see your insurance plan, please give our office a call for Special Plan at (203) 614-8998 and we will be more than happy to verify your coverage. Our office will make every effort to provide you with the finest care and the most convenient financial options.
